How Benefit Verification Works
To check your massage benefits, I need:

Once your benefits are verified, we’ll let you know what your plan may cover before scheduling.
Insurance coverage is not guaranteed until benefits are verified. Kaiser is not currently accepted.
Next Steps
After you submit the benefit verification form, I’ll check with your plan to see whether massage therapy benefits are available. Coverage varies by plan and may depend on your diagnosis, provider requirements, referral or prior authorization, visit limits, and remaining benefits.
If your insurer provides an estimate, I’ll explain the reported copay or other expected cost before you schedule. An estimate is not a guarantee of payment; your insurer makes the final decision, and you’re responsible for amounts assigned to you. If the plan does not cover the visit, I can explain current self-pay pricing. For auto accident or work injury claims, mention that on the form because the verification process may differ. Please use the form rather than sending insurance card details through social media.
